This is the 1st page of a (very) simple example Multi Page form built using Form1 Builder Software. It consists of four pages, with one input field per page and is based on the Quick Form structure with minimal formatting. You can give a it test run to see how it works. If you submit from the last page a copy of the form delivery email will be sent to the email address you provide on the third page (this field is set as required) and you will be redirected to the Form1 Builder Software page.
